thanks doc for the informative video. I have a question about taking master's programme in UK as I find that most of universities put in the requriements for the course let's say (orthopedics MCh in Teesside uni) is IELTS 7.5 . so do they actually only accept IELTS or is it a matter of not reporting that OET is valid?

This is actually a little painful to watch. She breaks down each section of the video part by part in order to (I assume) highlight inaccuracies, but contributes to the confusion: - she mentions that their graph is confusing and that GP takes 3 years. That is 3 years post foundation=5 years minimum. The real world context is important here - with the ever-growing competition from grads outside the uk, completion ratios are skyrocketing. Exams are also challenging. It is becoming increasingly rare that a doctor enters training directly from foundation and makes it through training without the inevitable stumbling blocks, which in all honesty she has probably experienced herself and is not highlighting. 5 years for Gp is the absolute minimum. - Doctors pay cut: yes, inflation has risen (and fallen) throughout the world to a varying degree, largely as a result of COVID related actions. However, this does not reflect the full story in the uk given the fact that austerity has been a major part of government policy for much longer and doctors pay has suffered for far longer than the cost of living crisis has been a feature of our economy. Equally, benefits that used to exist for doctors making our training more tolerable (eg free accommodation when sent across the country without a moments notice for training, like we signed up for the army) no longer exist. Her opinion here is, again, contributing to misinformation as it attempts to simplify something that she other does not understand or refuses to report on clearly. - Long hours: additional unpaid work is frequent. In the nhs, doctors that work longer than their contracted hours can notify HR via an internal system ‘the exception report’. This system however is deliberately laborious and requires your supervisor (who is also a doctor) to sign off on this extra hour - this leads to doctors not wanting to notify for fear of ramifications or questions about efficiency and skill. Anyone can see how this lends itself to a system that takes advantage of a huge swath of the critical workforce. Interestingly, in other government sectors eg police- the system is far more smooth and in other countries healthcare systems (eg Australia) the system is far more appropriate. Equally, rota’s are indeed awful and she has a truly questionable attitude here. Especially, considering she is constantly comparing the USA system where doctors are paid 5x uk norms and training is SIGNIFICANTLY shorter. She is not the only one that knows, has experience of and has family members who are doctors in the US. - Burnout+staff shortage: she seems to have an understanding of this although her company is effectively a part of the problem, not the solution. Heavy recruitment from countries outside the uk is not the long term answer and means auntie Margaret or uncle Jim literally has no idea what the doctor is saying to them because they came from a red country 10 days prior with no understanding of cultural norms or systems. - The fact that she specifies part of care as being “putting in a cannula” says a lot about her opinions. There should be no reason at all why a doctor needs to cannulate a patient or take bloods unless it is an arrest situation or extremely challenging access - despite what happens on a daily basis on the wards. - Competition ratios: she simply is wrong and that has played out clearly since this video was posted. Equally, CESR is not the answer either. This alternative training pathway leaves you in a difficult spot in so many other ways, including the difficulty in working overseas if you so choose. I could go on and on, but simply put, please save your thoughts to yourself and let the BMA be are spokesperson. Misinformation harms us and you clearly have an ulterior motive.
